What is In Vitro Fertilisation (IVF)?

In vitro fertilization also known as IVF is an assisted reproductive technique that makes it possible for a couple to conceive. The process is a surgical and medicinal one wherein the eggs collected from the ovaries are fertilized with the sperm in a specialized laboratory. The fertilized egg i.e. the embryo is then transferred into the woman’s uterus. The fertilization process takes place in a petri dish in which the embryo is allowed to grow for a few number of days in a protected environment before it is transferred.




What is the process involved in in vitro fertilization?

Before prescribing the IVF treatment, the woman will be asked to go undergo several tests that will help the doctor assess the ovaries and the uterine function. Then begins the fertilization process wherein the eggs and sperm are fertilized in a lab setting. If more then two embryos develop at the time of fertilization, the surplus embryo is frozen for use in subsequent cycles. Generally time of 15-20 days is required for one cycle of IVF, which is followed by 15 days of waiting period to do a pregnancy test. 




Steps involved in IVF Treatment

Step 1- Stimulation

The IVF process generally requires the woman to produce multiple eggs which is less likely as usually only one egg is produced during the monthly menstrual cycle. For this purpose, the woman is prescribed several fertility drugs to boost the egg production, which further helps in increasing the chances of fertilization. Regular ultrasound and blood tests are done to check the hormone levels.

Step 2- Egg Retrieval

This is a minor surgical procedure known as follicular aspiration. The patient is administered with a light sedative and with an ultrasound guide, eggs are collected with a hollow needle. After the retrieval procedure, the patient will be asked to wait for a few hours just to ensure everything is alright. Medication is prescribed to prepare the lining of the womb for embryo transfer. Spotting and abdomen pain is common at this stage. 

Step 3- Sperm Retrieval

Semen sample is collected from the male partner. Sperms showing maximum motility are selected. The fertilization process then takes place. However, if the doctor feels that the chances of fertilization are less then the sperm is directly injected into the egg through intracytoplasmic sperm injection. The sperm retrieved can be used for fertilizing a fresh egg or can be frozen for future use. 

Step 4- Embryo culture

To ensure that the egg is growing properly, the doctor will monitor the fertilized egg. The embryo takes at least 5 days to grow in a laboratory setting and this stage is known as blastocyst stage. Healthy looking embryos are identified. This is done visually and in some cases, genetic screening is done to check for genetic disorder if any.

Step 5- Embryo transfer

Typically, after 3-5 days after fertilization, the embryo is placed in the womb of the woman using a thin tube i.e. catheter. The pregnancy process begins when the embryo implants in the lining of the uterus and grows. At times more than one embryo is placed in a woman’s womb, which may lead to birth of twins or triplets.  The number of embryos that need to be transferred depends on the age of the woman and her health condition.

IVF success rate depends on a plethora of factors including woman’s age, lifestyle habits, medical history and cause of infertility. As per a study conducted by Society of Assisted Reproductive Technology (SART), the below success rates are reported depending on the age of a woman.

  • IVF is 41 to 43% successful for women aged under 35
  • IVF is 30to 36% for women aged above the age of 35 to 37
  • IVF is 23% to 27% for women aged above 38 to 40
  • IVF is 13% to 18% for women aged 40 and above
